കുട്ടിക്കാലത്തു മഴ തുടങ്ങുന്നതും സ്‌കൂൾ തുറക്കുന്നതും മിക്കപ്പോഴും ഒരേ ദിവസം ആവും.ആ മാസം തന്നെ മിക്കപ്പോഴും ഒരു മൂന്നാലു ദിവസത്തെ പനിച്ചു കിടപ്പും ഓർമ്മയിൽ ഉണ്ട്. സ്‌കൂളിൽ പോണ്ടാത്തതു കൊണ്ട് പുതപ്പിനിടയിൽ ചുരുണ്ടു കൂടിയുള്ള കിടപ്പിനൊരു സുഖം ഉണ്ട്. കുറഞ്ഞോ കുറഞ്ഞൊന്നും ആധി പിടിച്ച അമ്മയുടെ കൈകൾ നെറ്റിയിൽ തൊട്ടു നോക്കുന്നത് അറിഞ്ഞിട്ടും അറിയാതെ കിടക്കും.പ്ലാവില കൊണ്ട് ചൂടുള്ള കഞ്ഞിയും പപ്പടവും, അത് കഴിഞ്ഞും പായിലേക്കു. നാലാം ദിവസം സ്‌കൂളിലേക്ക് പോയാലും ക്ഷീണം വിട്ടു പോവാൻ പിന്നെയും രണ്ടാഴ്ച എടുക്കും. ഫ്ലൂ എന്ന വാക്ക് കേട്ടത് പിന്നെ എത്രയോ കഴിഞ്ഞു.  ടീവിയും ചാനലും ഒക്കെ വന്നപ്പോ ജൂൺ മാസത്തിലെ സ്ഥിരം കലാപരിപാടി ആയ ചാനൽ ചർച്ചകളും പനിക്കെടുതികളും ,ഓരോ കൊല്ലവും ആവർത്തനങ്ങൾ. രണ്ടായിരത്തി ഒൻപതിൽ ആണ് H1 N 1 എന്നയിനം ഫ്ലൂ ലോകത്താകെ പടർന്നു പിടിച്ചു മരണം വിതക്കുന്ന അറിഞ്ഞത്. ആ ഭീതി ഒരു പരിധി വരെ വിട്ടു മാറിയതായിരുന്നു. പക്ഷെ ഈ കൊല്ലം അതിന്റെ ഗൗരവം ഇത്തിരി കൂടി. ഇന്ന് വരെ ഉള്ള കണക്കെടുത്താൽ ഏകദേശം അറുന്നൂറ്റി അൻപതിലധികം കേസുകൾ ഈ കൂടിയ ഇനം ഫ്ലൂ ഉണ്ടായി കേരളത്തിൽ നാല്പതു ജീവനുകൾ കൊത്ത

9 year old girl   2 nd child born out of non-consanguineous marriage  with uneventful antenatal history, with a birth weight of 2.5 kg with nohistory of  NICU admission was discharged on post natal day  2. She was readmitted with frequent episodes of non bilious vomiting and Jaundice on post natal day  3 and diagnosed as duodenal web.  Duodeno-duodenostomy was done.  She was found to have some dysmorphic facial features which were subtle. Noticeable one was   bifid thumb on left hand. There was no significant malformations in the family. She was found to have abnormal cardiac findings on follow up, was investigated and found to have Osteum secundum ASD 8mm size . She was breast fed , was attaining weight and her developmental milestones at par with her peers.  At 4 years had and episode of hematemesis  There was  no other bleeding manifestations other than malena which followed next two days. At admission her vitals were stable,no features of hepatic enceph

12 year old boy presented to pediatrician with history of fever vomiting and abdominal pain, two days back. Pediatrician was my old student and he contacted me yesterday as this boy did nt pass urine for almost 24 hours and his renal function deteriorating. He developed conjuctival bleed and his coagulation parameters abnormal. He considered possibility of leptospirosis and was put on injection crystalline penicillin and ceftriaxone yesterday itself. He was send to us  daybefore yesterday night.  The investigation results done from outside were Total count 30000,with neurtophilia  Hemoglobin 14 gm/ dl Platelet 1.5 lakh Liver function test Total bilirubin 0.9 mg/dl,direct 0.2mg , SGPT 60 iu,SGOT 46 iu,Total protein normal  Coagulation parameters INR 3 , APTT 60  Blood urea 90mg/dl , Creatinin 4.3 mg/dl ESR 26 mms /hour  Lepto igM was negative On arrival ,  he was conscious , Blood pressure normal and capillary refill time 3 sec.periphery warm. Six year old boy brought with nasal regurgitation of fluids and nasal twang to voice since three days. Previously normal child went to sleep normally and when he woke up mother noticed the voice change and the nasal regurgitation of fluid while he was taking breakfast. 62 days old baby born to a young couple out of non consanguineous marriage. Elder sibling four years and healthy. Pregnancy period uneventful except gestational diabetes in third trimester which was well controlled throughout. First trimester there was no exposure to teratogens. Delivery at 38 weeks at hospital .Baby weight 2450 grams,not asphyxiated. Baby passed meconium and urine normally breast feeding established normally and was discharged on third day. She was gaining weight normally up to forty fifth day. From 45 days onward   mother noticed feeding difficulty in the form of stopping and resuming in between feeds.Baby took  longer time than normal on feeding and not getting enough sleep after feeding time. She also noticed baby was breathing faster during sleep and rest and gradually losing weight. discussion at this point Points useful for the clinical analysis here are  Onset and course of the problem.Whole problem started after one and a half months of age.

Six year old girl brought with yellowish discoloration of eyes and urine for the last  one and a half months. There was no fever any time in during the illness. No abdominal pain or discomfort. Overall course was gradual onset and slow progress. No significant bleeding manifestations, through bowel urine oral mucosa or skin. She was not constipated and stool was normally colored. No skin rashes or nodules. No joint pain or swelling. No alteration of sensorium or behavioral problems.Sleep was normal. No recent weight loss.     Born out of non consanguineous marriage to 32 year old father and 25 year old mother,elder girl child is healthy. No family history of jaundice, blood transfusion,abdominal surgery. No significant neurological / psychological  problems   in blood relatives. She was born at term after normal pregnancy. ജീവിത കാലം മുഴുവൻ ‘ ഇല്ലായ്മയെ കുറിച്ച് മാത്രം വേവലാതിപ്പെട്ടും പരാതി പറഞ്ഞും കലഹിച്ചും ഒക്കെ മുൻപോട്ടു പോവാനേ പറ്റിയുള്ളൂ.  സ്വന്തം കാര്യമല്ല പറഞ്ഞത്. എന്നെ പോലെ ,പരിമിത സാഹചര്യങ്ങളിൽ ആരോഗ്യ മേഖലയിൽ ജോലി ചെയ്യുന്ന ഒരു പാട് ആരോഗ്യ പ്രവർത്തകരുടെ കാര്യം. പല കാര്യങ്ങളെ കുറിച്ചും പഠിക്കും,പഠിപ്പിക്കും , അതേക്കുറിച്ചു വിദ്യാർത്ഥികളോട് ചോദ്യം ചോദിക്കും, ഉത്തരം പറയാത്ത ആളെ തോൽപ്പിക്കും. എന്നാൽ പറയുന്ന ഇക്കാര്യം ഒരിക്കൽ പോലും കണ്ടിട്ടില്ലാത്ത, ഉപയോഗിച്ചിട്ടില്ലാത്ത ആൾ ഇങ്ങനെ ചെയ്യുന്നതിന്റെ ശരികേട് ചിലപ്പോഴെങ്കിലും കുത്തി നോവിക്കും. അത്തരത്തിൽ ഉള്ള കാര്യങ്ങൾ ആണ് പുതിയ വാക്സിനുകളിൽ പലതും. ഇതേക്കുറിച്ചുള്ള വായിച്ചറിവ് ഏറെ ഉണ്ടെങ്കിലും പലതും ഉപയോഗിച്ചിട്ടില്ല. കണ്ടിട്ട് പോലുമില്ല.  ഒരു വര്ഷം മുൻപ് എന്റെ സുഹൃത്തു  ഡോക്ടർ ബാബു വിളിച്ചു ചോദിച്ചപ്പോ ആണ് മെനിഞ്ചോകോക്കൽ ബി (MENINGOCOCAL B ) വാക്സിനെക്കുറിച്ചു ആലോചിച്ചത് തന്നെ. മോന് അമേരിക്കയിലേക്ക് പഠനാവശ്യത്തിനു പോകും മുൻപ് വാക്സിൻ വേണ്ടിയിരുന്നു.എവിടെ കിട്ടുമെന്ന് ചോദിച്ചു വിളിച്ചതാണ്.അതിനു ശേഷം അദ്ദേഹം തന്നെ അത് അന്വേഷിച്ചു കണ്ടെത്തി കുട്ട

‘അമ്മക്ക് പ്രസവ വേദന മോൾക്ക് വീണ വായന’ എന്നൊരു ചൊല്ലുണ്ട്. അത് പോലെ ആണ് ചെലപ്പോ കുട്ടികളുടെ കാര്യങ്ങൾ.അവമ്മാരുടെ ചില വിക്രസ്സുകൾ കണ്ടാൽ ചിരി പൊട്ടും. അടുത്ത നിമിഷം അതിന്റെ ഗൗരവം ഓർമ്മ വരുമ്പോ ചിരി മായും. ട്രൗസറിന്റെ സിബ്ബ് കുടുങ്ങി ചോരയൊലിപ്പിച്ചു നിലവിളിച്ചു വരുന്നോരു ,  മറ്റു ചിലര് ചുണ്ണാമണിക്കു ചുറ്റും റബ്ബർ ബാൻഡെടുത്തു കെട്ടി വെച്ച് നീര് വന്നു വീർത്തു വരുന്നവർ, പൂച്ചയും നായയുമായി ചങ്ങാത്തം കൂടി വായിൽ കയ്യിട്ടു കടിവാങ്ങുന്നവർ. ഇതിലും രസമുള്ള കാര്യം ഓരോ ആഴ്ചയിലും ചെവിയിലോ മൂക്കിലോ എന്തെങ്കിലും ഒക്കെ എടുത്തു വെച്ച് “പറ്റിച്ചേ” എന്ന കള്ളച്ചിരിയുമായി വരുന്നവർ. അത്തരം ചിരിക്കു വക നൽകുന്നൊരു കാര്യവുമായി ഇന്ന് ഒരു വിളി വന്നു.വളരെ വേണ്ടപ്പെട്ട ഒരാളുടെ കുഞ്ഞുണ്ണി. അഞ്ചു വയസ്സുകാരൻ. റിമോട്ട് കൺട്രോൾ ബാറ്ററി ഒരെണ്ണം ഗുളിക പോലെ അങ്ങ് മിണുങ്ങി. തൊട്ടുകൂട്ടാൻ ഇത്തിരി അച്ചാറും കൂടി ആയാൽ ബഹു കേമം എന്ന മട്ട് ആളിപ്പോ അഡ്മിറ്റ് ആയി കെടപ്പുണ്ട്. പോകും വഴികളിലെ വാതിലുകൾ എല്ലാം മലർക്കെ തുറന്നു വെച്ച്  ,കാത്തിരിക്കുകയാണ്.  നാളെ പതിനൊന്നു മണിയോടെ ' അങ്ങേയറ്റത്തെ വാതിലിലൂടെ'  പു

    തൃശൂർ ജില്ലയിലെ സർക്കാർ- എയ്ഡഡ് സ്‌കൂളുകളിലെ   ഉച്ചഭക്ഷണ പദ്ധതിയെ കുറിച്ചൊരു ചർച്ചആയിരുന്നു. ബഹുമാനപ്പെട്ട എം പി ശ്രീ പി കെ ബിജുവിന്റെ അദ്ധ്യക്ഷതയിൽ കലക്ടറുടെ ചേമ്പറിൽ വിളിച്ചുകൂട്ടിയ യോഗം. ആരോഗ്യ വിദ്യാഭ്യാസ മേഖലയിലെയും  സപ്ലൈകോ ഫുഡ് കോർപ്പറേഷൻ ഓഫ് ഇന്ത്യ പഞ്ചായത്തു അധികൃതരും പങ്കെടുത്ത ചർച്ച. കുട്ടികളുടെ പോഷകാഹാരത്തെ കുറിച്ചുള്ള സാങ്കേതിക ഉപദേശങ്ങൾക്കു വേണ്ടി എന്നെ വിളിച്ചതായിരുന്നു. പദ്ധതിയുടെ അടിസ്ഥാന തത്വം "അറിവ് നൽകും മുൻപ് വിശപ്പകറ്റണം " എന്നത് . വിശപ്പ് ആറ്റുന്ന എന്ത് കൊടുത്താലും അറിവ് നേടാനായി സ്‌കൂളുകളിലേക്ക് കുട്ടികൾ എത്തും. ഇപ്പറഞ്ഞത് എഴുതി വായിച്ച അക്ഷരങ്ങൾ ആയിരുന്നില്ല.അനുഭവിച്ചറിഞ്ഞത്. പദ്ധതികളുടെ വിശദാംശങ്ങൾ ചർച്ച ചെയ്യുമ്പോൾ മനസ്സ് തെന്നി തെന്നി അൻപത്തി അഞ്ചു വർഷങ്ങൾ പുറകോട്ടു പോയി. ഒന്നാം ക്ലാസ്സു തൊട്ടു മേലോട്ടുള്ള ഓരോ ക്ലാസ്സുകളിലും ഉള്ള നാല് ചേച്ചിമാർ . അവരുടെ വിരൽ പിടിച്ചു സ്‌കൂളിൽ എത്തിയത് അക്ഷരം അറിയാനുള്ള ദാഹവും വിശപ്പുമായിരുന്നില്ല.  ഉപ്പുമാവിന് വേണ്ടിയുള്ള വിശപ്പു. മൂന്നു വയസ്സുകാരന് ക്ലാസ് മുറികൾക്കകത്തേക്കു പ്രവേശനം ഇല്ലാത്തതിൽ പര

ഒരു വർഷം മുൻപ് ഇങ്ങനെ ഒരു കുറിപ്പ് എഴുതാൻ ഞാൻ മുതിരില്ലായിരുന്നു. എഴുതുന്ന ഓരോ വാക്കും തപ്പിയെടുത്തു രോഗപ്രതിരോധ പദ്ധതികൾക്കെതിരെ വീശാൻ വാളുമായി കുറെ ഏറെ പേർ കച്ചകെട്ടി ഇവിടെ ഉണ്ടായിരുന്നു. വായിൽ തോന്നിയ വിഡ്ഢിത്തങ്ങൾ വെറുതെ വിളിച്ചു പറയുന്നവർ തൊട്ടു അധികാരത്തിന്റെ ഉന്നതങ്ങളിൽ വിലസുന്നവർ വരെ. ഇത്തിരി അഭിമാനത്തോടെ പറയാം എം. ആർ വാക്സിൻ കാമ്പയിൻ വിജയപ്രദമായി നടപ്പിലാക്കി കഴിഞ്ഞപ്പോഴേക്കും നമ്മൾ നേടിയെടുത്തത് ലക്ഷ്യമിട്ട എഴുപത്തി അഞ്ചു ലക്ഷത്തിൽ തൊണ്ണൂറ്റി ഒന്ന്  ശതമാനത്തിനും വാക്സിനേഷൻ നൽകി എന്നത് മാത്രമല്ല. അതിലും വലിയ മറ്റൊരു കാര്യം കൂടിയാണ്. എന്ത് മണ്ടത്തരവും ഇവിടെ ചിലവാകും എന്ന് കരുതിയിരുന്നവരെ ജനം പുറംകാലു കൊണ്ട് തട്ടിയെറിഞ്ഞിരിക്കുന്നു. ശാസ്ത്രാവബോധം ഉള്ള ,ശരി തിരിച്ചറിയുന്ന കേരളം നമുക്ക് തിരിയെ കിട്ടിയിരിക്കുന്നു. ആധുനിക വൈദ്യശാസ്ത്രത്തിന്റെ രീതിയിൽ ഏതൊന്നിന്റെയും ശരി തെറ്റുകൾ വിശകലനം ചെയ്തു തെറ്റ് തിരുത്തി / ശരികൾ മെച്ചപ്പെടുത്തി മുന്നോട്ടു പോവുന്ന രീതി. അതിനാണ് മുതിരുന്നത്.ഇത് വരെ ചെയ്തതെല്ലാം തെറ്റായിരുന്നു എന്ന ഏറ്റുപറച്ചിലായി വ്യാഖ്യാനിക്കപ്പെടില്ല എന്ന് ഉറപ്പുള്ളത്
